Registration Procedures
1. Your family must be registered in the parish. Click here for the Parish Registration Form.
2. A copy of the youth's Birth Certificate, Baptismal Certificate, and First Communion Certificate, if applicable, must be submitted at the time of registration for each new student to the program.
3. If applicable, the parents must provide the Custody section of their divorce, adoption, or guardianship papers.
4. Registration is open to baptized youth in grades 6-12. Younger students register with Religious Education.
5. Registration fees (per student) for Middle School youth are $80.00*. Registration fees (per student) for High School youth are $100.00*. These fees are due at the time of registration. For your convenience we can accept cash, check, credit cards, or debit cards.
6. A Catholic Youth Bible is required for both programs. If you currently don't have one, you may purchase a copy from the Youth Ministry office at registration.
7. Youth Ministry registration for the 2012-2013 school year is by appointment only. Please call 304-3010 or 304-3035 to schedule an appointment.
*We offer a family discount for multiple registrations in faith formation programs. Please ask for more information.
